Controlled substances may be purchased from Cerilliant Corporation by those registered with the Drug Enforcement Administration. We must have written confirmation in the form of a purchase order before shipment is made. In ordering controlled substances, you must submit: 1) a formal purchase order; 2) a copy of your DEA Form 223; and 3) a DEA Form 222 properly completed (Schedules I and II only).

Instructions appear on the back of the form. Do not alter or erase any portion of the DEA Form 222, including the information filled in by you. Please note that we can ship controlled substances only to the address shown on the form.

Remove the blue "Purchaser's Copy 3" and Purchaser's carbon only before sending the DEA 222 Form to Cerilliant Corporation, along with your purchase order and a copy of your DEA Form 223 Registration Certificate.
